Select the data region that matches your SurveySparrow account. US is the default. If your account was created in Europe choose EU, Asia-Pacific choose AP, and so on.

Your SurveySparrow Bearer token. Found under Settings > Apps & Integrations > API Keys. If you click Load Folders successfully, the key and region are saved in your browser's local storage so you don't have to re-enter them. Use "Clear saved credentials" to remove them.

Select which folder the survey should be created in. Click Load Folders to fetch folders from your account. If left blank, the survey lands in Personal.

ClassicForm supports all question types and is recommended for most surveys. NPS, CSAT, and CES are specialist survey types — they support only a limited set of question types.

Special handling

  • Bipolar matrix rows — write each row as Left label | Right label
  • Constant sum — list allocatable items under Options:
  • Consent — include Consent text: for agreement wording
  • Image-based choices — if a source PDF has picture options with no text, write the option as [Image: describe what you see]
